These simple jottings form are an easy way to get educators to jot things down throughout a session. Print on an A4 document and cut into 2, or even print multiple on an A4 page if desired. 
Educators can either then carry the slip of paper with them, or put it on a clipboard for easy access. 
 
Once completed, leadership, ed leaders etc can collate the notes to support circular planning, or educators can refer to it when writing more extensive notes.
